1st opg showing osteolytic area in region of 37.not very frank radiolucency.hard to detact radiologically without clinical information because of edentulous area.clinically patient was having a small ulceroproliferative growth in the same region.incisonal biopsy suggested moderately differentiated epithelial squamous cell carcinoma.patient was refered to higher centers for mangment of malignancy.
opg of same patient after 6 month showing displaced pathological fracture at angle of mandible.showing progressive nature of malignant process.opg was taken at aiims.
final opg u can see the severe destruction of cortex in form of diffuce radiolucency with ill defined borders.tooth also showing floating tooth appearance.patient again reported for biopsy slide for revaluation before the treatment.mri was also adviced for this patient .

my intesting cases
mohanpal--20 year old boy recalled from dept of ortho for his complaint regarding the spacing in his teeth...during routine check up ..we took a opg..in opg there were incomplete root formation in mandibular molars.some impacted teeth.retained deciduous..we shoot sum iopas..the picture was more clear..bone was normal but teeth were explaining the storey of sum thing unusual..but what was that..the very basic principle of osteosynthesis is BONE IS DYNEAMIC WHILE TOOTH IS STATIC...so we cant say anything by seeuing the bone..but there must be sum deficiency which cause disruption of growth during the development...
we took the medical history and general examination..patient was not intrested in activities like sports,tv,games...he left his school due to difficulty in memorising things and sum problem in his vision since 5 years.so we take opthalmology opinian..cataract surgery was done 3 years back..he was short in height..lean..nose was like mangaloid..frankly i thought about hypothyroidism..one striking part was his history of seizures for which he was taking carbazepine...since 7 years..he also gave history of seizures..so senario was totally full of confusion..we took medicine opinion ..physician give new insight he told it is tetany...
our teacher thought about sum rare syndrome..we sent the patient for calcium ,phosphate,alkaline phoshphatase...
